OLPH’s Women’s Guild 2023-2024 Grant Application is now open to ministries and organizations serving Our Lady of Perpetual Help Church or the community. Below you will find three (3) downloadable PDF files. Print out all three pages of the PDF document : The Application Brief, The Proposal for Funding, and The Funding Agreement. They must be filled out completely and returned to the Guild either by mail (address is on the letterhead) or brought to the Church Office (address is the same as the mail-in address).
